Medical dramas are no strangers to uncomfortable moments, but HBO Max’s The Pitt may have set a new bar this season — with help from a very convincing prosthetic.

In Season 2, Episode 2 of the hit hospital series, viewers watch ER doctors treat a patient suffering from priapism, a painful and dangerous condition involving a prolonged erection.

The cause: the patient, played by Christopher T. Wood, injected himself with double the recommended dose of an erectile dysfunction medication ahead of his 20th wedding anniversary.

While the scene is brief on screen, it left a lasting impression on the cast.

Sepideh Moafi, who plays attending physician Dr. Al-Hashimi, recalled spotting the prosthetic before filming even began. “I saw it in the hair and makeup trailer and thought, ‘Who’s having fun between shots?’” she said at the Season 2 premiere in Los Angeles. “Then I realized what it actually was. I still have nightmares about that scene.”

The procedure — a paracentesis to drain blood — was performed on a hyper-realistic model, with filming done on a closed set. Even so, not everyone was jealous of the actors involved. “I thank my lucky stars I wasn’t in that,” said Supriya Ganesh (Dr. Samira Mohan). “I don’t think I could have kept a straight face.”

Creator and showrunner R. Scott Gemmill says moments like these reflect the show’s commitment to portraying real emergencies without flinching. “There are some more terrible stories coming,” he teased. “Some of them are pretty nasty.”

Audiences clearly can’t look away. The Season 2 premiere drew 5.4 million viewers in its first days, a 200% jump from the series debut in 2025. The momentum continued at the Golden Globes, where The Pitt won Best Television Series — Drama, and Noah Wyle took home Best Actor.

With Season 3 already confirmed, Wyle hinted the show may avoid another major time jump. One thing, however, feels certain: The Pitt will keep pushing boundaries — even if it makes its own cast squirm.
